经皮肾穿刺微造瘘取石术与肾切开取石术治疗复杂肾结石临床分析

摘    要:目的探讨经皮肾穿微造瘘取石术与肾切开取石术治疗复杂性肾结石的临床疗效。方法将68例复杂肾结石患者随机分为观察组与对照组,观察组采用经皮肾穿微造瘘取石术手术治疗,对照组采用肾切开取石术手术治疗,比较两组患者手术时间、术中出血量、结石清除率、术后并发症发生率。结果观察组手术时间为67min,术中出血量为96mL,结石率为91.2%,术后并发症发生率为5.9%;对照组手术时间为92min,术中出血量为272mL,一次手术结石率为94.1%,并发症发生率为14.7%,观察组手术时间、术中出血量、术后并发症发生率少于对照组,结石清除率两组无显著差异。结论经皮肾穿刺微造瘘取石术疗效确切,安全有效,适于复杂性肾结石的手术治疗。

Abstract:Objective To investigate the therapeutic efficacy of minimally invasive percutaneous nephrolithotomy (MIPNL)and nephrolithotomy in complex renal calculi. Methods Sixty-eight cases of complex renal calculi were randomly divided into observation group and control group,renal calculi were moved by using MIPNL in the observation group and by using nephrolithotomy in the control group,and time of operation,amount of hemorrhage,clearance rate of renal calculi and incidence rate of complication were compared. Results Time of operation,amount of hemorrhage,clearance rate of renal calculi and incidence rate of complication were 67min,96mL,91.2% and 5.9% in the observation group and 92min,272mL,94.1% and 14.7% in the control group. The observation group showed less time of operation,less amount of hemorrhage and lower incidence rate of complication compared with the control group. Conclusion It is effective,safe and suitable to treat complex renal calculi by using MIPNL.
