清热泄浊法治疗痛风性肾病疗效观察

目的:观察清热泄浊法对痛风性肾病患者中医临床症状及实验室指标的影响。方法:50例痛风性肾病患者,按就诊先后顺序随机分为观察组30例和对照组20例,对照组采用别嘌呤醇和小苏打治疗,观察组在此基础上加用清热泄浊方治疗,疗程均为4个周。结果:观察组的总有效率90%,对照组65%。两组疗效比较差异有统计学意义(P<0.05)。治疗前后证候评分比较,观察组改善明显(P<0.01);两组间比较观察组治疗后证候评分显著优于对照组(P<0.01)。两组24 h尿蛋白定量、血尿酸(UA)、肌酐(SCr)、尿素氮(BUN)较治疗前均明显降低(P<0.01)。结论:清热泄浊法可明显改善痛风性肾病临床症状,降低其24 h尿蛋白定量、UA、SCr、BUN,且无明显不良反应。

Clinical Observation on Clearing Heat and Eliminating Turbidity Methods in Treating Gouty Nephropathy

Objective:To observe the effects of TCM clinical symptoms and laboratory indexes in patients with gouty nephropathy by clearing heat and eliminating turbidity. Methods:Total 50 patients with gouty nephropathy were selected and divided into 30 cases in the observation group and 20 cases in the control group according to the visiting order. The control group was treated with allopurinol and bicarbonate of soda while heat-clearing and turbidity-eliminating Decoction was added in the observation group on the basis of the control group for 4 weeks. Results:The total effective rates in the observation group and control group were 90% and 65% respectively. The difference of the total effective rates between the two groups was statis tically significant(P< 0.05). After treatment,the syndrome score in the observation group was significantly improved(P< 0.01). Compared with the control group,the syndrome score of the observation group was better than that of the control group after treatment(P< 0.01). After treatment,24 h urinary protein quantification,levels of blood uric acid(UA),creatinine(SCr) and blood urea nitrogen(BUN) of the two groups were significantly decreased(P< 0.01). Conclusions:Heat-clearing and turbidity-eliminating methods could improve the clinical symptoms of gouty nephropathy and reduce the 24-hour urine protein quantification,UA,SCr,BUN with no evident adverse reactions.
