断指再植术后的康复

目的阐述４０例１５７个断指再植术后进行系统康复治疗的方法和效果。方法断指再植成活后康复过程可分为早期、中期和后期３个阶段。早期康复目的主要是消炎消肿，采用超短波及紫外线照射，控制伤口感染，促进伤口愈合，改善血运，保证修复组织的正常愈合。中、后期康复重点是软化瘢痕、松解粘连，改善关节活动范围，恢复肌力及手的灵活性，恢复手的最佳功能，采用蜡疗、关节松动技术、运动疗法、作业疗法、感觉再教育和手夹板等方法。根据随访中手功能的情况，本组病例都择期做了肌腱手术等，加快了功能恢复。结果经术后６个月的随访，手部功能恢复，优良率达到８７．５％。结论断指再植成活为手功能恢复创造了可能性，康复治疗是这种可能性变为现实的保证。只有经过长时间的艰苦功能训练，才能使手的伤残减少到最低程度。

The follow-up observation on rehabilitation after replantation of digit-severed

Objective To state the methods and the effect of rehabilitation on 40 cases(157 fingers) of digits replantation. Methods The rehabilitation of digits replantation was divided into three stages: early, middle and late phase. The goals of the early phase were to promote the wound healing and to decrease the edema by ultrashort wave and ultraviolet. The emphases of the other phases were to soften the scar and relax adhesion,to improve the ROM of fingers by ADL exercise, hand occupational therapy, sensory reeducation, splinting and so on. According to the functional status at follow-up, secondary tendolysis operations were carried out. Results After six months of follow-up, the successful rate of hand functional recovery was 87.5%. Conclusion Digits replantation is only considered success when the function of the fingers has restored to a certain extent.Systematic rehabilitation is the important measures to assure success.
