食管癌术后心律失常危险因素的多因素分析

目的探讨对食管癌术后发生心律失常的危险因素以指导临床防治。方法采用病例对照研究方法,收集2002年1月~2005年6月期间在我院住院的食管癌手术患者资料,对术后心律失常可能的危险因素进行Logistic回归分析。结果多因素非条件Logistic逐步回归分析显示年龄超过60岁、FEV1/FVC<70%、术中失血超过600ml、术前半量放疗是食管癌术后心律失常的主要危险因素,术后镇痛是保护因素。结论对于高龄、术前放疗及术前肺功能下降的患者应做好充分的术前准备,选择合适的术式,术后常规镇痛。

Study on risk factors of cardiac anhythmia following operation of esophageal carcinoma

Objective:To investigate risk factors of cardiac arrhythmia folowing operation of esophageal carcinoma and search for the prophylaxis of postoperative cardiac arrhythmia.Methods:With methods of case-control study,an epidemiological survey of the cardiac arrhythmia folowmg operation of esophageal carcinoma from January 2002 to June 2005 was conducted. Logistic regression was performed. Results:The risk factors were identified by multifactor noconditional logistic regression,including the age older than 60y,FEV1/FVC<70%,blood loss during operation >600ml,preoperative semis radiotherapy. The protective factor was postopemtive analgesia.Conclusion:It is necessary to strengthen preoperative preparation and treatment for the patients of the age older than 60y,preoperative semis radiotherapy and preoperative pulmonary function descend. To keep a stable state of anesthesia during operation,to choose appropriate surgical approach of esophageal carcinoma and to take postoperative analgesia.
