诺和锐治疗急性心肌梗死合并应激性高血糖的临床疗效

摘    要:目的探讨应用微量泵静脉泵入诺和锐短期强化治疗急性心肌梗死(AMI)合并应激性高血糖的有效性和安全性。方法选择54例AMI合并应激性高血糖者,按数字随机法分为治疗组和对照组。治疗组(n=26)采用微量泵持续泵入诺和锐强化控制血糖,对照组(n=28)给予糖尿病饮食,避免输入含糖液体治疗。比较2组血糖变化及临床情况,包括胸痛缓解时间、溶栓再通率、并发症及预后。结果治疗组血糖平稳下降,胸痛缓解时间缩短,溶栓再通率高,心律失常、心力衰竭等并发症发生率较对照组低,2组比较差异具有统计学意义(P〈0.05)。结论 AMI合并应激性高血糖采用微量泵静脉泵入诺和锐短期强化治疗能提高AMI的治愈率,改善预后。

Effect of NovoRapid on stress-induced hyperglycaemia patients with acute myocardial infarction

Abstract:Objective To verify the effectiveness and safety of insulin aspart intensive therapy by the aid of microinfusion pump in acute myocardial infarction(AMI)patients accompanied with stress-induced hyperglycemia in a short term.Methods Fifty-four patients of AMI accompanied with stress-induced hyperglycemia were choosen and divided into two groups randomly. Patients in treatment group(n = 26) were treated with insulin aspart intensive therapy by the aid of micro-infusion pump,and control group(n = 28) was given special diabetics diets without the injection of sacchariferous liquid. The changes of blood glucose and clinical manifestation including chest pain relief time,recanalization rate,complications and prognosis were observed and analyzed. Results Compared with control group,the blood glucose was declined steadily,the chest pain relief time was shortened,the recanalization rate was higher,and the incidence of arrhythmia,heart failure as well as other complications was lower in treatment group(P〈0. 05) Conclutions NovoRapid intensive therapy by the aid of micro-infusion pump can enhance cure rate and improve prognosis in AMI patients accompanied with stress-inducde hyperglycemia in a short term.
