卵巢肿瘤患者血清CA125，CA199及CEA含量的检测及其意义

目的 研究CA125,CA199及CEA在卵巢肿瘤诊断及评价疗效中的作用。方法 分别以MEIA法及RIA法测定126例卵巢肿瘤（良性肿瘤62例、恶性肿瘤44例及交界性肿瘤20例）患者术及18例交界性或恶性肿瘤患者于术后3－4周血清CA125,CA199及CEA浓度。结果 ①卵巢交界性肿瘤及恶性肿瘤血清各项指标均高于良性肿瘤患者,差异有显著性（P＜0.05);②CA125与CA199对交界性肿瘤与恶性肿瘤的鉴别诊断意义不大,但恶性肿瘤中CEA明显高于交界性肿瘤;③Ⅰ期及Ⅱ期恶性肿瘤患者各项指标的阳性率均明显低于Ⅲ期及Ⅳ期患者,联合CA125和CA199检测可明显提高Ⅰ期和Ⅱ期患者诊断的阳性率;④CA125水平是判断卵巢癌临床治疗效果的重要指标。结论 CA125和CA199在卵巢肿瘤诊断及诊断、恶性肿瘤的分期和疗铲评价中有较大的价值,联合检测能提高Ⅰ,Ⅱ期卵巢癌的阳性率。

Measurement and significance of serum CA125, CA199 and CEA in patients with ovarian tumors

Objective To study the clinical values of serum CA125,CA199 and CEA in diagnosing ovarian tumor and in evaluating therapeutic effects. Methods MEIA and RIA was used to measure serum CA125,CA199 and CEA in 126 patients with ovarian tumor (benignant: 62 cases, malignant:44 cases and borderline tumor:20 cases) before operation and in 18 patients with borderline or malignant tumor 3-4 weeks after operation. Results ①CA125,CA199 and CEA were significantly higher in patients with borderline and malignant ovarian tumor than in those with benignant tumor. ②There was no significant difference between borderline and malignant tumor in CA125 and CA199 levels except CEA which was higher in patients with malignancy. ③Positive rates of all tumor markers in stageⅠandⅡpatients were lower than those in stage Ⅲ and Ⅳ patients.Combined measurement of CA125 and CA199 could increased diagnostic rates in stageⅠandⅡpatients.④CA125 and CA199 were an important indices index in evaluating therapeutic effects of ovarian cancer. Conclusion CA125 and CA199 have greater values in differentiation diagnosis,clinical staging and evaluation of therapeutic effect in patients with ovarian tumors.
