曲尼司特致肝损害和血尿

1名36岁女性患者，因面部皮疹口服曲尼司特胶囊0．1g,3；L／d抗过敏治疗，共20d，期间还使用了氯雷他定、西替利嗪等。皮疹好转后，继续服用曲尼司特8d。在服用该药的最后4d，患者出现了尿频、尿急、尿痛，伴肉眼血尿，轻度嗜睡。入院后实验室检查显示：ALT 716U／L，AST 263U／L，TBil 41．1μmol／L，DBil 20．0μmol／L，IBil 21．1μmol／L，GGT 246U／L，ALP 372．3U／L，血嗜酸性粒细胞0．66×10^9／L．尿常规示RBC满视野／HP。入院第二天患者出现发热（T38．5℃），胃纳减少，恶心，黄疸，轻度瘙痒，右肾区叩击有不适感。给予保肝、补液、抗感染等治疗，10d后患者病情逐步好转，治疗第24天后各项指标基本正常。

Hepatic injury and hematuria resulting from tranilast

A 36-year-old woman with facial rash received anti-anaphylactic treatment with tranilast capsules 0.1 g thrice daily for 20 days. During this period, she also took loratadine, cetirizine, etc. She continued to use tranilast for 8 days after her skin rash faded. However, within the last 4 days of tranilast administration, the patient developed urinary frequency, urgency, odynuria, gross hematuria, and mild lethargy. After admission, laboratory tests revealed the following levels： ALT 716 U/L, AST 263 U/L, TBIL 41.1μmol/L, DBIL 20.0 μmol/L, IBIL 21.1μmol/L, GGT 246 U/L, ALP 372.3 U/L. A blood routine test showed eosinophile granuloeyte eount of 0.66 ×10^9/L. An urinalysis revealed RBCs filled per high-power field. On the seeond hospital day, the patient developed a fever （ T 38.5 ℃ ） , anorexia, nausea, jaundiee, mild pruritus, diseomfortableness on pereussion at her right renal region. She was treated with liver-proteetive, anti-infeetive, and intravenous fluid therapy. Ten days later, the patient＇s condition started to improve. After 24 days of treatment, all laboratory values were within normal limits.
