妇产科人类免疫缺陷病毒感染/获得性免疫缺陷综合征患者的围术期处理

目的探讨人类免疫缺陷病毒(HIV)感染/获得性免疫缺陷综合征(AIDS)孕产妇围术期的处理方法,并以此评估HIV感染/AIDS患者行剖宫产手术的安全性。方法将住院分娩的HIV感染/AIDS患者64例作为观察组,另选正常孕产妇70例作为对照组。比较2组手术时间、术中出血量、术中并发症情况、术后排气时间、术后病率、切口愈合率及住院时间。结果 2组在手术时间、术中出血量、术中并发症情况、术后排气时间、术后病率、切口愈合情况、住院时间等方面比较差异均无统计学意义(P>0.05)。结论 HIV感染/AIDS的孕产妇,只要实施相应的母婴阻断药物治疗或抗病毒治疗,使CD4>250cells/μl,再实施剖宫产手术是安全有效的。

Efficacy of perioperative treatment in patients with the human immunodeficiency virus infection/the acquired immune deficiency syndrome of gynecology and obstetrics

Objective To investigate the perioperative treatment in maternal with the human immunodeficiency virus （HIV） infection/the acquired immune deficiency syndrome （ AIDS）, in order to assess the security of HIV infection/AIDS pa- tients with cesarean section. Methods 64 cases of maternal with HIV infection/AIDS of hospital delivery were set as observa- tion group,70 cases of normal pregnant women were set as control group. Compared the operation time, intraoperative blood loss, intraoperative complications, postoperative exhaust time, postoperative morbidity, wound healing and duration of hospitali- zation of two groups. Results The two groups in operative time,intraoperative blood loss,intraoperative complications, postop- erative exhaust time, postoperative morbidity, wound healing, there were no significant difference （ P 〉 0.05 ）. Conclusion HIV infection/AIDS maternal, maternal, if implementation of appropriate PMTCT drug therapy or antiviral therapy, CIM 〉 250cells/μl, then implementation of cesarean section is safe and effective.
