早期肠内营养联合益生菌用于脓毒血症治疗的效果观察

目的观察早期肠内营养联合益生菌治疗脓毒血症的临床疗效。方法采用前瞻性、多中心、临床随机对照研究方法，将56例脓毒血症患者随机分为早期给予肠内营养和益生菌组（观察组）和应用传统的标准肠外营养组（对照组），对两组患者的治愈率及住ICU时间、抗生素应用时间、机械通气时间进行对比研究，且治疗前及治疗后3、7、14d对患者进行急性生理学慢性健康状况评分系统Ⅱ（APACHEⅡ）评分及C反应蛋白（CRP）、降钙素原（PCT）监测情况进行比较。结果观察组患者治愈率高于对照组，其住ICU时间、抗生素应用时间、机械通气时间均短于对照组，两组治疗后APACHEⅡ评分及CRP、PCT检测值均有所降低，但观察组较对照组改善更为明显，差异有统计学意义（P〈0．05）。结论早期肠内营养联合益生菌的应用能够改善脓毒血症病情的严重度及预后。

Curative effect observation of early-stage enteral nutrition combining with probiotics in the treatment of septicopyemia

Objective To observe the clinical therapeutic effects of early-stage enteral nutrition combining with probi- otics in the treatment of septicopyemia.nethods Fifty six patients with septicopyemia were divided into observation group （early-stage enteral nutrition combining with probiotics group） and control group （standard parenteral nutrition group） by a prospective,multi-center, and clinical randomized controlled trial.The cure rate,intensive care unit （ICU） stay,antibiotic application time,and time of mechanical ventilation in both groups were compared.In addition,the acute physiology and chronic health evaluation Ⅱ （APACHE Ⅱ ） score and monitoring of CRP and PCT before treatment and 3 days,7 days,and 14 days after treatment were compared.Results The cure rate in the observation group was higher than that in the control group.The ICU stay,antibiotic application time,and time of mechanical ventilation in the obser- vation group were all shorter than those of the control group.The APACHE Ⅱ score after treatment and testing values of CRP and PCT in both groups decreased,but more remarkably in the observation group.All parameters in both groups displayed statistical differences after comparison （P〈0.05）.Conclusion Application of early-stage enteral nutrition combining with probiotics can improve the severity of septicopyemia and its prognosis.
