苯海索致226例不良反应文献分析

摘    要:目的探讨苯海索致药品不良反应(ADR)的一般规律及特点,为临床合理用药提供参考。方法检索“PubMed”系统、中国期刊全文数据库(CNKI)及万方数据库报道的226例苯海索不良反应病例,对患者的性别、年龄、原患疾病、既往史、药物过敏史、药物剂量、用药时间、ADR表现、处理及预后等进行统计分析。结果苯海索所致226例ADR病例中男性147例(65%),女性79例(359/6),原患疾病以精神分裂症(67.70%)、怕金森病(19.03%)多见。在98例有具体发生时间报道的患者中,ADR发生于7d以内者57例,占58.16%,发生于1个月以内者67例,占68.37%。苯海索所致ADR涉及人体多个器官系统,临床表现复杂多样,主要出现撤药症状、精神障碍、神经系统损害、全身性损害、泌尿系统损害、心率及心律紊乱、皮肤及其附件损害等。结论临床医师、药师应了解苯海索所致ADR的规律和特点,加强用药监测,以减少ADR的发生。

Abstract:Objective To explore the general patterns and characteristics of the adverse drug reaction (ADR) induced by trihexyphenidyl so as to provide references for rational drug administration. Methods The reports of 226 cases of adverse drug reaction were searched out from the PubMed System, China Na- tional Knowledge Infrastructure(CNKI) and Wanfang Data. The gender, age, primary diseases, past his- tory, allergic reaction history, drug dose, and time of drug use, clinical features of ADRs, management and prognosis were statistically analyzed. Results Among the ADR cases, 147 cases (65%) were male and 79 cases (35%) were female, Schizophrenia and Parkinson's disease were mostly discovered. In the 98 ca- ses with definite occurring time, 57 cases (58.16%) occurred within 7 days after taking the drug, 67 cases (68.38%) occurred within one month. The ADR affected multiple organ systems. The clinical manifesta- tions were various and were mainly discontinuation symptoms, mental discords, nervous system damage, systematic damage, urinary system damage, cardiac arrhythmia,skin and its accessory damage. Conclusion Doctors and pharmacists should well know the patterns and characteristics of the ADR induced by trihexyphenidyl and the drug should be used under strict monitoring to reduce the occurrence of serious ADR.
