血浆置换治疗重型肝炎的血生化指标变化及其意义

目的　探讨重型肝炎患者血浆置换治疗前后血液生化指标的变化及其意义。方法　测定 3 8例血浆置换后患者血浆置换治疗的血清丙氨酸氨基移换酶 (ALT)、门冬氨酸氨基移换酶 (AST)、L r 谷氨酰基移换酶 (GGT)、碱性磷酸酶 (ALP)、乳酸脱氢酶 (LD)、胆碱脂酶 (chE)、总胆红素 (TBil)、直接胆红素 (DBil)、前白蛋白 (PA)、总胆汁酸 (TBA)、总蛋白 (TP)、白蛋白 (ALB)、球蛋白 (GLOB)、白蛋白球蛋白比例 (A/G)、肾功能和电解质。结果　血浆置换后ALT ,AST ,ALP ,GGT ,LD ,TBil,DBil,TBA和GLOB明显降低 ,与治疗前比较 ,P <0 .0 0 1或P <0 .0 2。chE ,PA ,TP ,ALB ,肾功能和电解质无明显变化 ,与治疗前比较 ,P >0 .0 5。结论　血浆置换后肝脏功能明显改善 ,它为治疗重型肝炎提供了一个有效手段。

The Changes of Blood Biochemical Indexes of Severe Hepatitis Treated with Plasma Displacement and Their Significances

Objective To discuss the changes of blood biochemical indexes of severe hepatitis and their significances during the treatment with plasma displacement. Methods Biochemical indexes were measured in 38 cases before and after plasma displacement. Results Many indexes,such as alanine aminotransferase (ALT),aspartate aminotransferase (AST),alkali phosphatase (ALP),L gamma glutamyl transferase (GGT), actic dehydrogenase (LDH),total bilirubin (TBiL),direct bilirubin (DBiL),total bile acid (TBA) and globin (GLOB) in sera,decreased significantly after plasma displacement when compared with those before treatment ( P <0.001, or P <0.02),while other indexes such as cholinesterase (chE),pre albumin (PA),total protein (TP),albumin (ALB),some electrolytes and renal function,remained unchanged ( P >0.05).Conclusion It is suggested that liver function be improved after plasma displacement, which acts as an effective means to the treatment of severe hepatitis.
