新生儿破伤风患儿心肌酶谱、血气分析变化及临床意义

摘    要:目的:了解新生儿破伤风患儿急性期心肌酶谱、血气分析变化及临床意义,探讨心肌酶谱及血气分析变化对患儿预后的影响。方法:对33例急性期新生儿破伤风患儿及同龄对照组30例,分别检测各组的血清肌酸激酶(CK)及同功酶(CK-MB)、乳酸脱氢酶(LDH)、α-羟丁酸脱氢酶(α-HBD)及谷草转氨酶(AST),动脉血气分析,同时做临床及预后分析。结果:急性期新生儿破伤风与对照组比较,CK-MB、α-HBD明显升高(P<0.01),LDH及AST明显升高(P<0.05),剩余碱(BE)负值明显增大(P<0.01),心肌酶谱明显增高组(n=20)较非明显增高组(n=13)合并症增多(P<0.05),平均住院天数增加(P<0.05)。结论:新生儿破伤风患儿急性期存在不同程度心肌酶谱升高及代谢性酸中毒;心肌酶谱明显增高患儿合并症增多,住院天数增加。

Clinical significance and change of myocardial enzymes zymogram and blood gas a-nalysis in infantum tetanus
Abstract:Objective:To explore the clinical significance and change of myocardial enzymes zymogram and blood gas analysis in infantum tetanus.Methods:33 acute infantum tetanus neonates and 30 healthy neonates at the same age were selected,serum creatine kinasen(CK),isoenz(CK-MB),lactate dehydrogenase(LDH),α-hydroxybutyrate dehydrogenase(α-HBD),glutamicoxalacetic transaminase(AST),arterial blood gas analysis were detected.Results:The levels of CK-MB,α-HBD,LDH,AST and negative value for base excess(BE) in acute infantum tetanus neonates were higher than those in control neonates(P<0.05),complications incidence and hospital day in neonates with high myocardial enzymes zymogram were higher than those in neonates with low myocardial enzymes zymogram(P<0.05).Conclusion:High level of myocardial enzymes zymogram and metabolic acidosis are found in neonates with infantum tetanus;complications and hospital days increase for the neonates with high myocardial enzymes zymogram.
