老年住院病人下呼吸道感染病原菌分布与耐药性分析

[目的]了解老年住院病人下呼吸道感染病原菌的分布和药物敏感性的特点。[方法]对2003年1月～2005年12月间﹥60岁以上老年住院病人下呼吸道感染进行回顾性调查。[结果]3年间﹥60岁以上老年住院呼吸道感染病人共分离到371株病原菌,G＋菌占51.2%（190/371）,G-菌占44.7（166/371）,其中G＋菌以金黄色葡萄球菌、肺炎链球菌和肠球菌为常见;G-菌以大肠埃希菌、肺炎克雷伯菌、阴沟肠杆菌和铜绿假单胞菌为常见;同时还检出白色念珠菌33株、热带念珠菌5株;药敏结果显示G＋菌对万古霉素、克林霉素较为敏感,G-菌对亚胺培南、阿米卡星普遍较为敏感。[结论]老年住院病人下呼吸道感染的病原菌广、且耐药性严重,应加强耐药性监测,并要掌握病原菌及耐药性的新动态,对合理使用抗生素十分重要。

ANALYSIS OF PATHOGEN BACTERIA DISTRIBUTION AND DRUG RESISTANCE OF LOWER RESPIRATORY TRACE INFECTION IN OLD INPATIENTS

ObjectiveTo understand the characteristics of distribution and the drug resistance of pathogen bacteria of lower respiratory trace infection in old inpatients from 2003 to 2005.MethodsA retrospective investigation was conducted in inpatients aged above 60 with lower respiratory trace infection from 2003 to 2005.ResultsThe total strain of pathogen bacteria collected from 2003 to 2005 was 371.Pathogen bacteria included 190 Gram positive stains(51.2%),166 Gram negative strains(44.7%),33 Candida aibicans(8.9%)and 5 Candida Tropicalis(1.3%).Most of the Gram positive stains were Staphylococcus aureuse,Streptococcus pneumoniae and enteroccoccus which were only sensitive to Vancomycin and Clindamyein.And the Gram negative pathogens were mainly Escherichia coli,Klebsiella pneumoniae,Enterobacter cloacae and Pseudomonas aeruginosa which showed a narrow antibiotic sensitivity of Imipenem and Amikacin.ConclusionsThere are kinds of apthogen bacteria strains leading to lower respiratory trace infection of old inpatients.And most of them show severe drug-resistance to antibiotics.So,it is very important that doctors should master the newly development in this field to better use of antibiotics.
