肺腺鳞癌外科治疗44例分析

目的 探讨肺腺鳞癌外科治疗的临床特点。方法 分析经手术切除,病理确诊的44例肺腺鳞癌资料,进行数据统计分析。结果 与同期肺鳞癌、腺癌比较直接侵犯率分别为79．4％、25．5％和36．1％;淋巴结阳性率分别为63．6％、40．6％ 和33．3％;术中所见的跨叶侵犯和淋巴结表现与术前x线及CT表现分别为40．9％、11．3％和63．6％、40．9％,均有显性差异。结论 本组结果表明：肺腺鳞癌在直接侵犯、淋巴结转移程度上明显高于鳞癌和腺癌,术前影像学检查很难表现出其实际病变范围,肺腺鳞癌的预后明显差于肺鳞癌和腺癌。

Surgical treatment of adenosquamous carcinoma of the lung

Objective To study the features in surgical treatment of adenosquamous carcinoma of the lung.Methods 44 cases of adenosquarmous carcinoma of lung which received surgical removal and was confirmed pathologically have been analysed in the past 10 year. Data base were statistically analyzed by using EPI Info 6.04 software. Results The outcome of surgery for adenosquamous carcinoma of lung compared with result of squamous cell carcinoma and adenoarcinoma in same period was 79.4% 25.5%and 36.1% respectively in direct invasion of the adjacent structures;63.6%?40.6% and 33.3%respectively in lymph node metastasis; The results of intraoperative findings of adenosquamous carcinoma compared with results of preoperative evaluation in X ray or Computed Tomographic(CT) examination of chest was 40.9%and 11.3%respectively in invasion of interlobar;63.6%and 40.9%respectively in lymph node metastasis,which are statistically significant. Conclusions Adenosquamous carcinoma of lung is significantly higher in direct invasion of the adjacent structures and lymph node metastasis than squamous cell carcinoma and adenocarcinoma.The evaluation of preoperative X ray and CT can't determined the invasion of interlobar and nodal status accurately.The prognosis in patients with adenosquamous carcinoma of The lung was poorer than squamous carcinoma and adenocarcinoma of the lung.
