中心静脉导管用于胸腔积液闭式引流效果观察

目的观察中心静脉导管用于治疗胸腔积液闭式引流的可靠性和临床意义。方法对恶性胸腔积液组35例、结核性胸膜炎组11例患者采用中心静脉导管胸腔留置行闭式引流，观察其操作用时间、胸水排净时间、留管时间、不良反应、堵管、继发感染情况。结果置入导管操作用时3～8min，平均5．1min；胸水排净时间3～12d，平均5．4d；导管保留时间为5～24d，平均11．5d；不良反应轻且发生率低为4．3％（2／46）；发生堵管3例；所有病例未发生引流部位或胸腔内感染。结论中心静脉导管用于胸腔积液闭式引流方便、安全有效。

Application of Central Venous Catheter in Closed Drainage of Pleural Effusion

Objective To Observe the reliability and clinical significance of central venous catheter in closed drainage of pleural effusion. Methods The closed drainage of pleural effusion was performed in 35 eases of malignant pleural effusion and 11 eases of tuberculosis pleurisy by indwelling central venous catheter. The operating time, hydrothorax evacuating time, catheter retaining time, adverse reactions, obstruction of catheter and secondary infection were observed. Results The operation time of intubations was 3 to 8 minutes, an average of 5.1 minutes;pleural effusion evacuating time was 3 to 12 days,an average of 5.4 days; catheter retention time was 5 to 24 days, an average of 11.5 days ; adverse reactions was mild and the incidence rate was low （4.3%） ;blockage only occurred in 3 cases. The infection of chest or drainage locality did not occurred in all cases. Conclusions Central venous catheter is convenient.safe and effective in closed drainage of uleural effusion.
