旋转DSA在颅内动脉瘤诊疗中的应用

目的：评价旋转DSA在颅内动脉瘤诊疗中的应用价值。材料与方法：对12例自发性蛛网膜下腔出血患者行常规正、侧位和各种斜位及旋转DSA。在工作站寻找显示动脉瘤最佳角度,将C臂机架转到最佳工作角度,在此角度栓塞动脉瘤。结果：在12例自发性蛛网膜下腔出血患者中检出动脉瘤,其中微小动脉瘤2例,选择诊疗的10例中8例致密填塞,2例次全填塞,2例微小动脉瘤未予治疗。结论：旋转DSA有助于发现颅内动脉瘤、微小动脉瘤,能为动脉瘤栓塞治疗提供最佳的工作角度,使动脉瘤栓塞更加安全可靠;可对高度怀疑颅内动脉瘤患者目标血管不做常规脑血管造影直接行旋转DSA检查,栓塞。

The Application of Rotational DSA on the Diagnosis and Treatment of Intracranial Aneurysms

Purpose： To evaluate the utility of rotational DSA on the diagnosis and treatment of intracranial aneurysms. Materials and Methods： Rotational DSA was performed in 12 patients with spontaneous subarachnoid hemorrhage by frontal,lateral and ruarious oblique projecttions including rotational DSA. The perfect visionangie of aneurysms could be found on the workstation and make the C arms turning to the proper angle to have aneurysms embolized totally. Results： 12 spontaneous aubarachnoid hemorrhage cases were found with aneurysms including 2 micro - aneurysms. Ten cases were undertaken selective embelizafion with 8 of total occlusion and the other one of partial occlusion, The patient with micro - aneurysms didn＇t receive embolization. Conclusion： Rotational DSA can improve in finding intracranial aneurysms and micro - aneurysms and provide the best procedural angulation for embolization with more safe; we don＇t want to make normal cerebral angigraphy, and directly make a rotational DSA examination, embolism of objective blood vessel for the patients of intracranial aneurysm who pay much attention to suspection by us.
