阻塞性黄疸术后肾功能衰竭的原因分析

目的：为了解阻塞性黄疸术后肾功能衰竭的原因，更好地预防和治疗术后肾衰。方法：对山西省人民医院近十年来收治的手术治疗的梗阻性黄疸60例临床资料进行分析。60例中，肾衰9例，其中死亡3例。结果：阻塞性黄疸患者术后发生肾功能衰竭的主要因素有：围手术期循环血量不足；患者的年龄；术前的高胆红素血症；术中出血量和血压波动。结论：为预防术后肾功能衰竭，应采取术前给予适当输血、补液，纠正低血容量，维持水电解质及酸碱平衡；需尽量缩短手术时间；术后注意保证肾脏的有效循环血量；用引流和利尿的方法降低血清胆红素；合理选用抗生素；补充营养以维持机体的营养状况和正常代谢等。
