2型糖尿病患者口服葡萄糖耐量试验中血清游离脂肪酸动态变化与大血管病变的关系

摘    要:
目的研究2型糖尿病患者口服葡萄糖负荷下血清游离脂肪酸(FFA)的动态变化与大血管病变的关系。方法51例T2DM患者:大血管病变组28例,非大血管病变组23例;正常对照组15例;测口服葡萄糖耐量试验(OGTY)中0、60、120、180min各点血清FFA值,比较3组FFA动态变化,并就FFA的影响因素作相关分析。结果各时间点FFA水平:大血管病变组较正常对照组显著升高(0、60、120和180min,P均〈0.05,P〈0.01);较非大血管病变组升高,仅空腹P〈0.05。非大血管病变组较正常对照组升高,180min末达统计学意义,余各点P〈0.05。60minFFA下降幅度:正常对照组较非大血管病变组明显下降(P〈0.05)。结论OGTY中,糖尿病大血管病变组各点FFA显著升高,三组FFA动态变化存在差异,FFA与糖尿病大血管病变联系紧密。

Abstract:
Objective To investigate the relationship between the change of serum free fatty acid(FFA)and macrovascular complications in type 2 diabetes(T2DM).Methods The FFA levels of OGTT at 0min,60min,120min and 180min were measured in 28 T2DM patients with macrovascular complications [DM+MV(+)],23 T2DM patients without macrovascular complications [DM+MV(-)],and 15 normal control subjects(NC).The correlation between FFA and other parameters were analyzed.Results FFA levels of the DM+MV(+) were significantly higher than those of the NC at all time points(0 and 60min,P<0.01,120 and 180min,P<0.05).The FFA levels at 0 60 and 120min of the DM+MV(-)were significantly higher than those of the NC(P<0.05).The FFA levels of the DM+MV(+) was higher than those of the DM+MV(-)(P<0.05).The percentage suppression of FFA at 60min from the fasting value of the NC was higher than that of the DM+MV(-)(P<0.05).Conclusion During the OGTT,the FFA levels of the DM+MV(+)were significantly higher at all time points,the dynamics of FFA alterations of the three groups are different.The FFA levels and T2DM macrovascular complications are tightly correlated.
