摘    要:目的评价磁共振增强扫描结合波谱成像对脑肿瘤术后损伤、残留与复发的MR表现。方法应用飞利蒲AchiEva1.5TMR常规增强结合’H-MRS对23例脑肿瘤术后的病灶进行回顾性分析。观察病变强化区的主要代谢产物,并计算代谢物的比值(NAA、cho、Cr、Cho/Cr、Cho/NAA、NAA/Cr),比较各代谢物在肿瘤复发或残瘤的意义。结朵23例脑肿瘤中肿瘤残留、复发17例,反应性损伤及放疗后坏死6例。其中经病理组织学证实12例,经临床及影像学随访证实11例。肿瘤残留、复发病变区表现为术区或颅内出现软组织肿块,增强后不均一强化。MRS显示Cho峰升高,Cr及NAA峰降低;Cho/Cr、Cho/NAA比值均较正常健侧升高,,而反应性损伤术区呈均一的环状强化,MRS显示Cho峰不高,Cr及NAA峰降低,Cho/cr、Cho/NAA比值较正常健侧不高。结论一H-MRS与增强MRI结合能更好地提高脑肿瘤术后反应性损伤、放疗后坏死与残留及复发的鉴别诊断,对指导临床治疗方案的选择具有指导意义。

Evaluation on Single-voxel Magnetic Resonance Spectro-scopy in Detecting Patients with Brain Neoplasm after Treatment

Abstract:Objective To evaluate the manifestations of magnetic resonance enhanced scan- ning plus spectro-scopy for postoperative damage, residual and recurrence of patients with brain neoplasm. Methods 23 patients with brain neoplasm were analyzed retrospectively by enhanced scanning plus 1 H-MRS of Philip Achieva 1.5T MR. The value of NAA, Cho, Cr were observed ,and the ratio of Cho/Cr, Cho/NAA,NAA/Cr were calculated. The value of the metabolites in the residual or recurrence of tumor were compared. Results 17 of 23 lesions were patients with residual or recun'ence of tumor,6 were patients with reactive injury and necrosis after radiotherapy. Of 23 patients, 12 were proved pathologically,l I were proved by clinical or hnaging follow-up. Soft-tissue mass in the operative or intracra- nial area were showed in the residual or recurrence area of tumor, with non-uniformed strengthening after enhancement. MRS showed increased Cho,and decreased Cr,NAA.The ratio of Cho/Cr and Cho/NAA were higher than that of the normal side. The unifonlled rim enhancement were showed in the reactive injury area. MRS showed nornaal value of Cho, and decreased Cr and NAA. The ratio of Cho/Cr and Cho/NAA were not higher than that of the normal side. Conclusions ~H-MRS plus enhanced MRI scanning can make better differential diagnosis tbr residual, recurrence, reactive injury or necrosis after radiotherapy of brain neoplasm. It is helpful for the selection of clinical treatment
