摘    要:
目的:探讨机械通气在急性左心衰竭治疗中的应用时机选择。方法:将32例采用早期机械通气的急性左心衰患者作为治疗组,28例未采用早期机械通气的患者作为对照组,观察两组患者机械通气后0.5h、1h、3h的呼吸频率(RR)、动脉血氧分压(PaO2)、心率(HR)、平均动脉压(MAP)等变化情况,及患者住院病死率。结果:两组患者机械通气后0.5、1、3h的RR、PaO2、HR、MAP较通气前有明显改善(P<0.05);治疗组中30例缓解,2例死亡,对照组中20例缓解,8例死亡,两组住院病死率差异有统计学意义(P<0.05)。结论:在救治急性左心衰患者时尽早应用机械通气能有效改善低氧血症、提高抢救成功率。

Abstract:
Objective:To investigate the occasion of mechanical ventilation in patients with acute left heart fail- ure. Method: A total of 60 patients with acute severe left heart failure were randomly treated with early mechanical ventilation (treated group, 32 cases) or without early mechanical ventilation (control group, 28 cases). The respira- tory rate, PaO2,heart rate and mean arterial pressure after receiving 0. 5 h, 1 h, 3 h treatment were observed. Re- sult: The respiratory rate,PaO2,heart rate,and mean arterial pressure of patients in treatment group after 0.5 h, 1 h, 3 h treatment were improved compared with those of patients in control group,The differences were significant (P〈0.05). 2 cases died in treatment group,while 8 cases died in control group. The mortality were significantly different (P〈0.05). Conclusion: Early mechanical ventilation treatment can improve the hypoxemia and reduce the mortality of patients with acute severe left heart failure.
