洛赛克预防脑出血昏迷并发症应激性溃疡出血的临床观察

目的 :观察洛赛克预防脑出血并发应激性溃疡出血的疗效。方法 :随机将 62例脑出血昏迷病人分为静脉应用洛赛克、西米替丁及胃管注入洛赛克片剂 (胶囊 ) 3组。全部病人置入胃管抽取胃液测量用药前后持续 7天的 p H值及潜血反应。结果 :胃液 p H值由用洛赛克前 1.3 7± 0 .70经用药后升至 5 .0 6± 0 .78(P<0 .0 1)。洛赛克静脉注入与胃管注入两组比较无差异 (P>0 .0 5 ) ,洛赛克两组与西米替丁组比较差异显著 (P<0 .0 1)。洛赛克静脉注入与胃内注入预防脑出血应激性溃疡出血疗效一致 ,总有效率均为 90 .4 8% ;应用西米替丁组有效率则为 65 % ,两组比较 P<0 .0 5。结论 :洛赛克能有效地预防脑出血并发应激性溃疡出血。

Clinical Observations on the Therapeutic Effect of Losec in Preventing Cerebral Hemorrhage and Coma Complicated by Bleeding Stress Ulcer

Purpose: To observe the therapeutic effect of losec in preventing cerebral hemorrhage complicated by bleeding stress ulcer. Methods: 62 cases of cerebral hemorrhage and coma were randomly divided into 3 groups: (1)Intravenous drip of losec; (2)Administration of losec by gastrogavage; (3)Intravenous drip of cimetidine. The gastric juice was drawn from all of the patients to measure a week successivepH value and the occult blood reaction before and after administration of drugs. Results: pH value of thegastric j uice increased from 1.37 ± 0.70 (before administration losec) to 5.06 ± 0.78 ( P < 0.01 ) (after).There was no difference between the first and second groups (P <0.05). Comparison of the first and thesecond groups with the third group is P < 0.01. The therapeutic effect of the first group is the same as thesecond group (90.48%). The total effective rate is 90.48%. The effective rate of the third group is 65%.The ratio between two groups is P < 0.05. Conclusion: Losec can effectively prevent cerebral hemorrhagecomplicated by bleeding stress ulcer.
