视路白质纤维束磁共振扩散张量成像的初步应用

摘    要:目的探讨应用DTI技术对视神经纤维束和视路成像的可能性和应用价值。方法采用GE1.5T Signa HD磁共振。纤维束示踪成像软件为日本东京大学Masutani开发的DTV2和Volume One软件,共检查30例,其中15例志愿者,15例患者。结果30例中,视交叉、视束和视放射均可良好显示。有8例志愿者和6例患者可清晰显示完整的视交叉与视神经,其余未能完整显示视神经。4例眼眶肿瘤可见视神经受压移位,部分形态改变,提示视神经损伤。2例炎症由于未侵犯视神经,视神经纤维束显示未见异常。结论应用DTI技术观察视神经及全视路神经纤维束的改变,显示眼眶肿瘤与视神经的关系,对提高视路疾病的显示和诊断具有明显的意义。

Abstract:Objective To discuss the feasibility and value of diffusion tensor imaging (DTI) to reveal optic nerve and visual pathway. Methods GE 1.5T Signa HD MR system, DTV2 and Volume One software of Tokyo University were utilized. Thirty cases, including 15 volunteers and 15 patients were enrolled in the study. Results Optic chiasma, optic tract and optic radiation were all well demonstrated. But only in 8 volunteers and 6 patients (including 4 orbital tumor and 2 inflammation patients),the optic nerves were successfully revealed. Shift of orbital nerves with partial malformation caused by oppression was displayed in 4 orbital tumor patients, so the injury of orbital nerves were suggested. Optic nerves in 2 inflammation patients were demonstrated as normal because inflammation did not encroach on optic nerves. Conclusion Diffusion tensor imaging can demonstrate the visual pathway, especially optic nerve and the relationship between orbital nerve and tumor, which offers a new method to research the disorders of optic nerve and visual pathway.
