96例高原肺水肿患者临床分析

摘    要:
目的探讨高原肺水肿的临床特点,并总结其诊治经验。方法对我院1998年1月至2005年10月收治的96例高原肺水肿(HAPE)患者临床资料进行回顾性分析研究。结果快速进入高海拔地区、上呼吸道感染、寒冷为HAPE的好发因素。HAPE起病急,病情进展迅速,心电图上表现以窦性心动过速、右室电压增高、顺轴向转位为主,而心动超声图亦有左室射血分数、肺动脉压升高等表现。胸部CT检查可有助于早期诊断HAPE,而采取的综合治疗总有效率达到93.8%。结论高原肺水肿应早诊断、早治疗,早期有效的以氧疗为基础的综合治疗是关键。

Abstract:
Objective In order to improve the early diagnosis and therapy of High-Altitude Pulmonary Edema (PBC), we investigated clinical characteristics of HAPE. Methods 96 patients with HAPE, who were diagnosed and treated in People hospital of Diqing Prefecture from Jan.1998 to Oct.2005, were retrospectively analyzed. Resluts Going into high-altitude area, upper respiration tract infection and cold were common inducement of HAPE. HAPE was acute and developed rapidly. Electrocardiograph of HAPE usually presented sinus tachycardia, hyper pressure on right ventricle and clockwise axes locomotion. And echocardiograph of HAPE presented elevated left ventricular ejection fraction and elevated pulmonary artery pressure, et al. CT of breast could help to the early diagnosis of HAPE and the deficiency of integrated therapy of HAPE were 93.8%. Conclusion HPAR should be diagnosed and treated early, and the pivotal treatment was the efficient integrated therapy based on oxygenic therapy.
