高龄老年患者心脏起搏器植入手术的安全性评价

摘    要:目的评价高龄老年患者植入心脏起搏器的安全性。方法127例高龄老年患者(≥80岁,高龄老年组)与298例老年患者(60~79岁,老年组),从起搏器植入指征、起搏模式、起搏电极参数、静脉穿刺成功率以及围手术期并发症等方面进行对照研究。结果高龄老年组双束支或三束支阻滞、病态窦房结综合征合并房室传导阻滞以及心房颤动的发生率均明显高于老年组(11.81%vs6.04%,13.39%vs7.05%,33.86%vs24.50%);高龄老年组单腔(VVI)模式起搏比率明显高于老年组(33.86%vs23.83%);高龄老年组锁骨下静脉穿刺的一针成功率明显低于老年组(56.69%vs68.12%);两组患者在围手术期并发症、起搏电极参数、起搏电极径路等方面均无统计学差异。结论高龄老年患者的心脏起搏器植入手术是安全的,其风险与普通老年患者相仿。

Abstract:Objective To evaluate the safety of pacemaker implantation in senile elderly patients.Methods One hundred twenty-seven cases of senile elderly group(≥80 years)and 298 cases of older group(60~79 years)were compared with the indication of pacemaker,pacing mode,pacing electrode parameter,venepuncture ratio and perioperative complications.Results In senile elderly group,the ratio of bifascicular block and trifascicular block,sick sinus syndrome combined with atrio-ventricular block and Atrial fibrillation were much more than older group(11.81% vs 6.04% ,13.39% vs 7.05% ,33.86% vs 24.50% );ratio of single-chamber pacing mode was higher(33.86% vs 23.83%);but the stitch puncture ratio of subclavian vein was much less than older group(56.69% vs 68.12%).No significant difference in perioperative complications,pacing electrode parameter and entrance.Conclusions The pacemaker implantation in senile elderly patient is safe,and the risk is similar to older man.
