胆道镜在肝内外胆管结石治疗中的应用

目的 总结13年来胆道镜在肝内外胆管结石治疗中的经验. 方法 分析近13年来胆道镜治疗肝内外胆管结石的病例.在1125例接受治疗的患者中,A组,术中胆道镜取石者536例,B组,术后经胆道引流管窦道治疗者517例,C组,术后切开永久性皮下通道治疗者72例. 结果 A组配合肝段切除结石取尽率93.7%,B组结石取尽率84.1%,C组结石取尽率90.2%. 结论 胆道镜在肝内外胆管结石的治疗中具有重要作用.但应强调外科手术相结合,争取在术中结合病灶切除等方法提高结石取尽率.手术建立永久性皮下通道,对复发结石的治疗和减少再次手术有重要作用.

Application of the choledochoscopic treatment for patients with intrahepaticduct stones

Objective Investigation the effect of fabric-choledochoscope in treatment of Calculus of hepatbile duct. Methods Analysis of 1125 cases in Calculus of hepat-bile duct therapied with fabric-choledochoscope. Group A includes 536 cases removed calculus through fabfic-choledochoscope in operation; Group B contains 517 cases therapied through bile draln-tract; Group C involves 72 cases treated through cutting permanency hypodermic tact. Results The rate of complete removed calculus is 93.7% in group A, assisted with hepatic segments excision; The rate of group B is 84.1%; The rate is 90.2% in group C. Conclusions Fabriccholedochoscope in treatment of Calculus of hepat-bile duct plays a important role; To have assisted with hepatic segments excision could increase the rate of complete removed calculus; Building permanency hypodermic tact is important in treatment of remnant calculus or recurring calulus.
