腓骨固定在严重胫腓骨粉碎性骨折治疗中的作用

目的:探讨腓骨固定在严重胫腓骨粉碎骨折治疗中的作用。方法:1998年1月-2003年1月治疗80例严重胫腓骨粉碎性骨折患者,男54例,女26例;年龄12~76岁,平均34·5岁。42例仅固定胫骨,38例同时固定胫腓骨。比较两组的骨折愈合时间、并发症发生率、患肢膝、踝关节功能优良率。结果:随访2年以上,42例仅固定胫骨者的平均骨折愈合时间为7·3个月,发生并发症16例,患肢膝、踝关节功能优良率分别为78·6%和71·4%;38例固定胫腓骨者的平均骨折愈合时间为3·8个月,发生并发症2例,患肢膝、踝关节功能优良率分别为85·9%和94·5%。两组比较,差异均有显著性意义(P<0·01)。结论:对严重胫腓骨粉碎性骨折,固定胫腓骨可简化手术,促进骨折早期愈合,减少畸形愈合,降低并发症和获得更好的患肢功能。

The effects of the fibula internal fixation for treating serious tibial and fibular comminuted fracture

Objective:To discuss the effects of the fibular internal fixation in the treatment of serious tibial and fibular comminuted fracture.Methods:Eighty patients (54 male and 26 female with average age of 34.5 years ranging from 12 to 76 years) with serious tibial and fibula comminuted fracture were treated.Among the all 80 patients 42 cases (group A) were treated with fixation of only tibia and 38 cases (group B) with fixation of both tibia and fibula.After the treatment the average fracture healing time,the rate for the occurrence of complications and the rate of excellent or good function of the knee and the ankle joint were compared between the two groups.Results:All patients were followed-up for two years at least.In group A,the average fracture healing time was 7.3 months,the complication occurred in 16 cases and the rate of excellent or good function of the knee joint was 78.6% and that of the ankle joint was 71.4%.In group B,the average fracture healing time was 3.8 months,the complications occurred in 2 cases,the rate of excellent or good function of the knee joint was 85.9% and that of the ankle joint was 94.5%.There were significantly statistical differences in all the parameters above between the two groups(P<0.01).Conclusion:In the treatment of serious tibial and fibula comminuted fracture,the fibular internal fixation can simplify the surgical procedures,promote the fracture healing,decrease the formation deformity,reduce the rate of complication occurrence and reach better recovery of joint function.
