新生儿窒息酸碱紊乱及阴离子间隙分析

摘    要:目的 探讨新生儿窒息酸碱紊乱的特点及其意义。方法 测定68例窒息新生儿的动脉血气及血清电解质,计算阴离子间隙(AG)值及应用预计代偿值计算式来判断三重酸碱紊乱(TABD),以明确酸碱紊乱的类型。结果 68例中单纯酸碱紊乱16例,二重酸碱紊乱44例,三重酸碱紊乱5例。高AG35例,正常AG27例,低AG6例。结论 新生儿窒息时糖酵解增加,大量乳酸堆积,导致高AG状态的代酸是窒息酸碱紊乱的最根本原因。除测动脉血气外应同时测定血清电解质及推算AG值。处理时应改善机体缺氧状态,保护和支持各脏器功能,使用碱性药物。

Abstract:Objective To explore the manifestation and significance of acid-base imbalance in newborns with asphyxia.Methods The types of acid-base imbalance were determined in 68 newborns with asphyxia through detecting arterial blood-gas and serum electrolyte,calculating their AG values and judging the TABD by prediction blood-gas compensation formula.Results There were 16 cases with simple acid-base imbalance,44 cases with BABD,5 cases with TABD,35 cases with high AG levels,27 cases with normal AG levels,and 6 cases with low AG levels.Conclusion The primary causes of the acid-base imbalance in neonatal asphyxia are the increased glycolysis,large amount of lactic acid accumulation and high AG acidosis.Therefore the serum electrolyte level should also be detected besides the calculation of AG and the determination of blood-gas.During treatment,effects should be made to improve anoxia,protect and support the function of each organ,and be cautious with alkaline drugs.
