超声造影评价宫颈癌放化疗效果的临床价值

摘    要:
目的探讨超声造影(CEUS)评价中晚期宫颈癌放化疗效果的临床价值。方法对28例中晚期宫颈癌患者放化疗前后进行CEUS检查,分析其表现及时间一强度曲线(TIC)部分参数的变化。结果28例患者放化疗前测量肿瘤大小的范围13.2~98.3mm。,浸润深度(18.9±10.5)mm,7例见阴道侵犯,1例见膀胱侵犯。放化疗后9例肿块完全消退,17例部分消退,2例无变化;浸润深度(9.7±7.8)mm,与治疗前比较具有统计学意义(P〈0.01)。3例阴道侵犯完全消退;1例膀胱侵犯未见明显变化。TIC参数在治疗后表现为显影时间(AT)、达峰时间(TTP)及增强时间(△T)延长,增强强度(EI)及单位时间强度变化(k)明显减少,与治疗前比较均有统计学意义。结论CEUS可以为中晚期宫颈癌的临床治疗效果评价提供可靠信息。

Value of Contrast-enhanced Ultrasound in Evaluting Effects of Chemotherapy and Radiotherapy on Cervical Carcinoma

Abstract:
Objective To explore contrast-enhanced ultrasound ( CEUS ) in evaluting effects of chemotherapy and radiotherapy on local advanced cervical carcinoma. Methods Twenty-eight cases of local advanced cervical carcinoma underwent CEUS with SonoVue before and after chemotherapy and radiotherapy.The sonographic characteristics and the perfusion quantified parameters of time-intensity curve (TIC) were analyzed. Results In the twenty-eight cases before treatment , the measurement of tumor size was 13.2 to 98.3mm2, infiltration depth was ( 18.9 ~ 10.5 )mm, seven cases were infiltrated into vagina, one case was infiltrated into urinary bladder.Aftter treatment, nine cases showed complete remission ( CR ), seventeen cases showed partial remission ( PR ), two cases showed stable disease ( SD ), infiltration depth was ( 9.7 + 7.8 ) mm. Compared with the data before treatment the difference was significant (P 〈 0.01 ) , three cases with vagina infiltration showed complete regression, one case with urinary bladder infiltration had no change.Compared with the data before treatment, the arrival time ( AT ), the time to peak ( TTP ) and the enhancal time ( A T ) was increased obviously while the enhanced intensity ( EI ) and the unit time strength changes ( k ) were lessened obviously ( the difference was significant ). Conclusion CEUS can provide useful evidence for evaluting effects of chemotherapy and radiotherapy on local advanced cervical carcinoma
