不同剂量丙种球蛋白治疗川崎病的临床疗效研究

目的观察不同剂量丙种球蛋白治疗川崎病的临床疗效。方法选取2008年1月～2010年1月来我院就诊的川崎病患者52例作为观察对象，随机分为对照组[常规剂量，400mg／（kg·d）]和观察组（大剂量，2g／kg）两组，每组26例，比较两组治疗前后血沉（ESR）、PLT、CRP的变化情况，同时比较两组的临床症状消退时间及住院时间。结果两组治疗后ESR、PLT、CRP均较治疗前明显降低，且观察组降低程度较对照组更明显（P〈0．05）。观察组治疗后的退热时间、黏膜充血消退时间、手足肿胀及淋巴结肿大消退时间、住院时间均较对照组短（P〈0．05）。观察组无一例发生冠状动脉受损。结论大剂量丙种球蛋白治疗川崎病的效果明显优于常规治疗，可以明显改善患儿的临床症状、缩短住院时间，值得临床推广和应用。

Different Doses Gamma Globulin Treatment of Kawasaki Disease

Objective To observe the clinical efficacy of different doses of gamma globulin in treatment of Kawasaki disease. Methods Fifty-two patients with Kawasaki disease as objects of observation, were randomly divided into control group （ conventional dose, 400 mg/ （ kg ·d ）） and observation group （high dose, 2g/kg） of 26 cases in each. Erythrocyte sedimentation rate （ ESR ）, PLT, CRP changes, and clinical symptoms subsided time and hospital stay of the two groups were compared before and after treatment. Results After treatment, ESR, PLT, CRP was significantly lower than before treatment in both groups, and the dereased level of the observation group was significantly better than the control group （P 〈 0.05 ）. After treatment, the fever subsided time, mucous membrane hyperemia subsided time, hand and foot swelling and lymph nodes swelling subsided time, hospital stay of the observation group were shorter than the control group （P 〈 0.05 ）. The observation group had no case of coronary artery damage. Conclusion High-dose gamma globulin treatment of Kawasaki disease is better than conventional treatment, can significantly improve clinical symptoms in children, has shorter hospital stay, and should be promoted and applied.
