两种手术方法在白内障术中应用的比较

目的 观察隧道式小切口环形撕囊和信封式截囊劈核法在白内障治疗中的应用效果。方法 对172例172眼白内障按手术随机顺序分为两组,分别采用上述两种方法进行手术并比较其结果和特点。结果 88例隧道切口吲哚靛青绿(ICGA)染色环形撕囊法手术中,连续环形撕囊成功80例占90.91％,人工晶状体完全植入囊袋者82例;占93.18％,角膜出现轻度水肿10例占11.36％。与之相对比,84例小切口信封式截囊劈核法人工晶状体植入囊袋者70例占83.33%(P<0.05),角膜轻度水肿6例占7.14%(P>0.05),均用10-0尼龙线缝合。结论 两种手术方法具有相应的特点和难点,环形撕囊法人工晶状体囊袋内植入率高而不需缝合,接近超声乳化手术效果。

The comparison of two different methods in cataract operation

Objective To investigate two different methods in cataract operation in the basic county hospital and estimate its feasibility. The first method includes tunnel-incision, CCC (continuous circular capsulorhex-is) dyed by ICGA, IOL endocapsule implanting without suture placing. The second method includes small-incision, envelope capsulectomy and cataract nuclear chopping with suture placing.Methods 88 and 84 patients (172 eyes) were arranged into two groups randomly and accepted the operations by two different ways respectively. The results and characteristics of the two operations were analyzed. Results In the first method,CCC under ICGA dying was successfully in 80 eyes(90.91%) and 82 eyes(93.18%) had IOL endocapsule implantation completely, 10 eyes (11.36%) had cornea edema.In the second method, 70 eyes (83.33%) had IOL endocapsule implantation completely (P < 0.05) and 6 eyes (7.14%) had cornea edema (P > 0.05). Also every patient in the second method had the incision sutured with 10-0 nylon suture . Conclusion The two methods had their difficulties and characteristics, the first had higher rate of IOL endocapsule implantation without suture and had the nearly effect of that of PHACO.The ophthalmologists in basic hospital should chose the cataract operation method carefully according to the conditions.
