玻璃体腔内注射康柏西普联合玻璃体切除术治疗增殖性糖尿病视网膜病变的临床效果

目的：观察玻璃体腔内注射康柏西普联合玻璃体切除术治疗增殖性糖尿病视网膜病变（ PDR）的临床效果。方法将临床确诊为 PDR的患者79例79眼纳入本研究。依据术前是否行玻璃体腔内注射康柏西普将患者分为试验组和对照组：试验组40例40眼、对照组39例39眼。试验组术前3d玻璃体腔内注射康柏西普0.05mL（0.5mg），然后行玻璃体切割术，对照组直接行玻璃体切割术，术后随访12个月。比较两组术后视力、平均手术时间、术后眼压、术后并发症。结果试验组较对照组术后视力明显提高，分别为0.25±0.18和0.15±0.12，有统计学意义（P<0.05）。平均手术时间试验组为25.26±10.31min，而对照组为41.12±13.54min（P<0.05）。试验组术后3d眼压0眼升高，而对照组眼压Tn+1为5眼，占12.8%（P>0.05），术后随访期内，试验组发生牵拉性视网膜脱离行再次手术后视网膜复位为1眼，而对照组再出血8眼，其中4眼出现牵拉性视网膜脱离需要再手术。结论增殖性糖尿病视网膜病变的患者，术前玻璃体腔注射0.5 mg康柏西普可以显著提高视力，缩短手术时间，减少术后并发症的发生率，而对术后腿压的影响不大。

The Clinical Effect of Intravitreal Injection of Conbercept Combined with Vitreous Body Resection in the Treatment of Proliferative Diabetic Reti-nopathy

ABSTRACT:OBJECTIVE To observe the clinical effects of vitrectomy assisted with intravitreal injection of con-bercept for proliferative diabetic retinopathy (PDR).METHODS The clinical diagnosis of 79 cases with 79 eyes PDR patients included in this study.According to whether preoperative intravitreal injection conbercept heap divided the patients into experiment combination control group:Experiment group 40 cases(40 eyes) ,Control group 39 cases (39 eyes).The experimental group treated with conbercept intravitreal injection,0.5mg,3d before vitrectomy,and control group directively treated with vitrectomy.The follow-up time was 12 months.Main outcome measures were postoperative visual acuity,mean surgical time,intraocular pressure,incidence of iatrogenic retinal breaks were recor-ded,and the effects of drug residues in normal blood vessels.RESULTS The visual acuity of the experimental group and the control group was significantly improved,and there was statistical significance ( P<0.05).Mean surgi-cal time was 25.26 ±10.31 min in experimental group vs 41.12 ±13.54 min in control group ( P<0.05 );Experi-mental postoperative intraocular pressure 0 eye 3d increased,whereas the control group 5 eyes Tn+1,accounted for 12.8%,there was not statistical significance (P>0.05).Postoperative follow-up period,1 eye occurred traction reti-nal detachment,again after the operation of retinal reattachment in test group,while in the control group rebleed 8 eyes,of which 4 eyes appear tractional retinal detachment need reoperation.CONCLUSION In this clinical study, we found in patients with PDR,preoperative intravitreal injection of 0.5mg conbercept can significantly improve visu-al acuity,short operation time,reduce the incidence of postoperative complications,but had little effect on intraocular pressure after operation.
