双极"D"形电切圈套器的体外实验研究和临床应用体会

目的　评价双极“D”形电切圈套器的应用效果及安全性 ;方法　对体外乙状结肠假息肉用双极“D”形电切圈套器和单极椭圆形圈套器行电切 ,并对其结肠壁标本进行病理学检查 ,比较临床效果 ;结果　双极“D”形电切圈套器具有操作简单、方便等特点 ,10例中出现迟发性出血 1例 ,即时性大出血 1例 ,与用单极椭圆形圈套器比较时间和能量需求差别无显著性 ;病理学检查提示 ,用双极者组织损伤明显小于用单极者 ,损伤达肌层者分别为 10 %和 90 % ,边缘损伤范围分别为 (0 .4 2± 0 .13)mm和 (1.4 7± 0 .5 8)mm ;结论　双极“D”形电切圈套器用于电外科治疗具有组织损伤小、操作简单和方便、相对安全性高等优点 ,但临床应用尚需积累更多的经验

In Vitro Study on "D" Shaped Bipolar Snare and Its Application in Clinical Practice

Objective The clinical effect and safty of the "D"-shaped bipolar snare and its clinical application were studied.Methods In vitro electric resections were performed on pseudopolyps by "D"-shaped bipolar snare or the monoplar snare, then the resected samples of these two approaches were compared in pathological and clinical aspects. Results The "D"-shaped bipolar snare is easy and safe to operate clinical application. Among those 10 cases treated with this procedure, delayed hemorrhage and immediate bleeding occurred in 1 case respectively. There was no significant difference between the bipolar and the monopolar snare procedures in time and energy requirement of operation. The pathological study suggested that the tissue damage resulted from the bipolar snare was significantly less than that from the monopolar snare. Conclusion The bipolar snare as a tool used in electrosurgery is relative safety. It is not only easy to operate but causes little tissue damage as well. But further experiences should be accumulated in its clinical practice.
