109例支气管活检肺腺癌的临床病理分析

目的观察肺腺癌的临床病理特征、诊断及鉴别诊断。方法对109例肺腺癌患者的临床及病理资料进行回顾性分析,并根据2011年肺腺癌新分类对109例肺腺癌标本重新进行镜下观察和免疫组化评估。结果肺腺癌临床主要症状为咳嗽、气短及胸痛,CT扫描常表现为实性的结节、毛玻璃样斑块和实性/毛玻璃样斑块。肺腺癌主要组织学类型包括:浸润前病变、微浸润性病变、浸润性病变。免疫表型:肺腺癌组织中CK7阳性率为100%,TTF1阳性率为72.2%,未见CK5/6表达。结论肺腺癌的诊断及鉴别诊断主要依靠临床资料、组织学特征及免疫表型。

Clinicopathological study of 109 cases of adenocarcinomas by transbronchial lung biopsy

Purpose To study the clinicopathologic characteristes,the diagnosis and differential diagnosis of adenocarcinomas of lung.Methods The clinical and pathological data of 109 cases were reviewed.The tumors were retrospectively analyzed by light microscopy and immunohistochemistry according to international multidisciplinary classification of lung adenocarcinoma in 2011.Results The initial syndrome of lung adenocarcinoma in the most patients was cough,shortness of breath and chest pain.By CT screening,aadenocarcinoma often showed solid nodules,ground glass opacities and solid/ground glass opacities.Histologically,these tumors has some histological subtypes.The major individual subtypes are preinvasive lesion,minimally invasive adenocarcinoma and invasive adenocarcinoma.The tumor cells were positive for CK7 and negative for CK5/6 in all cases.The positive rate of TTF1 was 75.2%(82/109).Conclusions The diagnosis and differential diagnosis of adenocarcinomas of lung depends on its clinical data,histopathology and immunohistochemistry.
