埋线结合艾灸辨证治疗雄激素源性脱发的疗效观察

目的观察埋线结合艾灸治疗雄激素源性脱发的临床效果。方法治疗前检测纳入研究的300例患者的血清睾酮(T)及雌二醇(E2)水平,并计算其比值。同时完善临床观察记录表。再根据治疗的先后顺序,按照随机数字表随机分入对照组与治疗组。对照组给予非那雄胺口服。治疗组在双侧肾关穴、明黄穴及足三里穴埋线,并用温灸盒艾灸少腹部。治疗时间均为3个月,随访6个月。患者在治疗1,3,6个月后再次检测血清T、E2的水平,并计算其比值。结果治疗1,3,6个月后2组患者T/E2比值气血两虚型有显著性差异(P均<0.05),肝脾湿热型与肝肾阴虚型无显著性差异(P均>0.05)。治疗组中气血两虚型患者的治疗效果明显优于对照组,其他2型则与对照组的治疗效果没有显著性差异。结论埋线配合艾灸治疗气血两虚型雄激素源性脱发疗效确切,而对肝脾湿热型及肝肾阴虚型的雄激素源性脱发效果并不明显。

Clinical observation of the therapy of catgut embedding and moxibustion on androgenetic alopecia

Objective It is to observe the clinical effect of the therapy of catgut embedding and moxibustion on androgenetic alopecia.Methods Before the treatment,all the patients' serum testosterone level(T) and blood serum estradiol level(E2) were determined,and the ratio of T/ E2 was calculated.At the same time,the clinical observation records were finished.All cases were randomly divided into treatment group and control group according to their sequence.The control group's patients were given finasteride tablet.The treatment group's patients were treated with the therapy of catgut embedding on shenguan point and minghuang point and zusanli point,then with the therapy of moxibustion on hypogastrium.The two groups were treated for three months,and followed up for six months.The patients' serum T and E2,and to calculate the ratio of T/ E2 were determined and the ratio of T/ E2 was calculated again in 1,3,6 months after treatment.Results There was significant difference in T/E2 value between the patients with Qi-blood deficiency in both groups after 1,3,6 months' treatment,but no significant difference was found between the ones with liver-gallbladder damp-heat and liver-kidney Yin deficiency in both group.The curative effect of the patients with Qi-blood deficiency was much better than that of the control group,but the difference in the effect of the other two syndromes in treatment group was not significant compared with that of the control group.Conclusion The therapy of catgut embedding and moxibustion has a good curative effect on androgenetic alopecia of Qi-blood deficiency type,but its effect on the disease of liver-gallbladder damp-heat and liver-kidney Yin deficiency types is not obvious.
