颌骨动脉瘤性骨囊肿的影像学表现

摘    要:目的:认识颅骨动脉瘤性骨囊肿的影像学表现,提高术前诊断率。方法:回顾性分析3例经病理证实的颌骨动脉瘤性骨囊肿的平片、CT和MRI影像特点。3例均行平片、CT、MR检查,部分做冠状和矢状位扫描。结果:平片上2例骨膨胀性改变,l例溶骨性改变;CT显示病灶边缘薄层骨壳环绕及骨性间隔;MRI:TlWI上l例可见液液平,l例显示纤维间隔。与软组织界面显示在CT、MRI均为光滑锐利。结论:动脉瘤性骨囊肿的影像表现虽不能明确诊断,但结合临床应考虑到该病变的可能。当然,最终的明确诊断仍需组织病理学检查,尤其在有原发病变时。

Abstract:Objective:To identify the findings of aneurysmal bone cyst(ABC) invelving the jaws and to improve the diagnosis of ABC presurgical resection.Methods:3cases of ABC comfirmed by histopathology were studied restrospectively.Routine plain film,CT and MR examination were performed in all 3 cases.Some were performed with coronal and/or saggital position.Results:Radiograph:2typical expansile and 1 osteolytic apperance;CTfindings:2 lesions surrounded by thin wall of bone.1bony septums were visible within the lesion.1 osteolytic lesion with sharp smooth and slightly sclerosis margin;MRI findings:All lesions were surrounded by a thin well-defined low singal rim on both T1-and T2-weighted images.Fluid level and thin fibrous septums were demenstrated on T1-weighted image in 2 different cases.Conclusions:Although these findings are not necessarily pathognomonic for ABC,they are highly suggestive especially combined with clinical findings.Certainly, the decisive diagnosis depends on histopathology and it is essential to identify if there is a previous lesion.
