大黄甘草汤在重症急性胰腺炎患者中的治疗观察

摘    要:目的探讨大黄甘草汤对重症急性胰腺炎(SAP)的治疗效果及作用机制。方法将69例SAP患者随机分为2组:常规治疗组(CT,n=34),大黄甘草汤灌胃组(DG,n=35)。在常规治疗的基础上,DG组加用大黄甘草汤灌胃。观察两组患者的心率、呼吸、血压、肝肾功能、氧合指数(PO2/FiO2)、血氧饱和度(SaO2)、血常规、血浆电解质、葡萄糖及C-反应蛋白;并行APACHEⅡ评分及腹痛缓解时间、肛门排气时间、住院时间观察;分别于患者入院时,治疗后24h、48h,96h取血测TNF-α,IL-10水平。结果治疗后24h、48h、96h,与CT组比较,DG组在白细胞、血糖、血钙、肝肾功能指标、APACHEⅡ评分均明显改善(P0.05);TNF-α水平降低(P0.05);IL-10水平升高(P0.05);APACHEⅡ评分与TNF-α呈正相关(r=0.816,P0.01),与IL-10呈负相关(r=-0.704,P0.01),TNF-α与IL-10呈负相关(r=-0.819,P0.01);腹痛缓解时间、肛门排气时间、住院天数明显改善(P0.05);DG组在治疗后急性肺损伤、胸/腹腔积液、肠麻痹/梗阻、败血症等并发症及并发症总发生率较CT组明显改善(P0.05),其余并发症及死亡率呈下降趋势,无统计学意义(P0.05)。结论大黄甘草汤治疗SAP可明显改善患者肝肾功能及全身情况,缩短住院时间,减少并发症。
