骨肿瘤与肿瘤样病变的CT诊断价值

目的:探讨CT在骨肿瘤及肿瘤样病变诊断中的应用价值。方法:回顾分析了经手术病理CT引导下穿刺活检和临床随诊证实的63例骨肿瘤及肿瘤样病变的CT资料并与平片对比。结果:①由于CT有较高的密度分辨率,结合平片能提高骨病变的定位准确率。本组病例CT结合平片的定位准确率为100%。②CT能早期发现病变。6例平片显示阴性的病例,CT扫描发现了病灶。③CT能提高骨病变的定性准确率。本组病例的定性准确率为90%(57/63),对某些骨病变CT能判断其病变的生长方式,扩展方向,对治疗方案提供有价值的资料。④CT结合平片必要时行CT引导下骨穿活检是很有价值的骨病诊断方法。6例CT和平片难以定性的病灶,CT引导下骨穿5例最终确诊。结论:CT对骨肿瘤及肿瘤样病变的诊断具有很高价值。但不能取代X线平片,二者结合,再辅以CT引导下骨穿活检,完全可满足骨肿瘤及肿瘤样病变的诊断问颢。

Diagnostic Value of CT in Bone Tumor and Tumoroid Lesion

Objective:To investigate the value of CT in diagnosis of bone tumor and tumoroid lesion.Methods:The CT and plian film were retrospectively analysed and compared in 63 cases of bone tumor and tumoroid lesion comfirmed by biopsy and clinical follow-up visit.Results:Combined with plain film,CT with higher density resolution could improve the rate of fixation of bone lesion.In this group,The accurate rate of fixation of CT combined with film was 100%.CT could detect early lesion which was negative in plain film in 6 case.CT improved the accurate rate of qualitation in bone lesion,which was 90% in this group.CT also provided valuable evidence for evaluating the growth pattern and direction in extension in certain bone leision.The bone puncture and biopsy guided by CT was valuable diagnostic methods on bone lesion.5 of 6 cases of uncertain lesion was diagnosed by bone puncture guided by CT.Conclusion:CT plays an important role in diagnosis in bone tumor and tumoroid lesion,but CT can not replace film examination.CT combined with plain film radiography or bone puncture and biopsy can solve the problems in diagnosis in bone tumor or tumoroid lesion.
