摘    要:目的对比评价即时检验(POCT)法和化学发光法检测肌钙蛋白I(cTnI)的临床检测效能和可靠性。方法收集60例临床急性心肌梗死(AMI)患者,分别采用POCT法和化学发光法对cTnI进行测定,对两种方法的测定结果进行比较和线性相关分析。结果在cTnI阴性和高浓度时,POCT法无法检测,而化学发光法能检测;在低和中浓度时两种方法之间没有统计学差异(P〉0.05),但具有很强的相关性。结论POCT法快速简便,适于急诊快速诊断AMI患者;化学发光法灵敏度高,特异性和准确性好,适于AMI患者的临床确诊和治疗监测。

Abstract:Objective To evaluate and compare the clinical performance and reliability of POCT and chemiluminescence method in the detection of troponin I. Method Troponin I level was detected with both chemiluminescence and POCT methods in 60 acute myocardial infarction patients. The results were compared and analyzed with linear correlation analysis. Results When the troponin I level was obviously negative and high, results shown by two methods were significantly different, and only chemiluminescence could detect it. As for moderate and low level of troponin I the difference was insignificant(P〉0.05), with high linear correlation. Conclusion Being quick and simple, POCT is suitable for emergency and initial screening. Chemiluminescence is beneficial for the diagnosis of acute myocardial infarction and monitoring because of its high sensitivity and good accuracy.
