异丙酚对糖尿病患者压力感受反射敏感性的影响

目的观察不同浓度异丙酚对糖尿病患者压力感受反射敏感性(barorefiex sensitivity,BRS)的影响。方法选择40例择期乳腺癌根治手术患者,年龄50~65岁,ASAⅠ或Ⅱ级。其中20例糖尿病患者和20例非糖尿病患者,分为糖尿病组(D组)和非糖尿病组(N组)。分别在麻醉诱导前(T0)、异丙酚靶控浓度5 mg/(kg·h)(T1)、异丙酚靶控浓度10 mg/(kg·h)(T2)时测定BRS值。结果 D组较N组T0、T1、T2时点BRS值均降低(P<0.05)。与T0比较,两组患者其余时点BRS值均降低(P<0.05);与T1比较,两组患者T2时点BRS值降低(P<0.05)。结论异丙酚对压力感受反射敏感性的抑制呈浓度依赖性,且对糖尿病患者影响较非糖尿病患者明显。

Effect of propofol on carotid sinus baroreflex sensitivity in diabetes mellitus

Objective To investigate the effect of different concentration of propofol on carotid sinus baroreflex sensitivity in diabetes mellitus. Methods Forty ASA I or lI patients aged 50-65 years, were divided into normal （N） and dia- betes mellitus （D） groups,twenty nondiabetics and twenty diabetes mellitus who was going to be radical mastectomy for breast cancer. BRS was measured before induction of anesthesia （TO）, when concentration of propofol reached 5 rag/ （kg-h）（T1）, when concentration of propofol reached 10 mg/（kg.h）（T2）. Results The BRS of D group was lower than that of N group at TO, T1 and T2 （P 〈 0.05）. Compared with TO, the BRS of two groups in other points were lower （P 〈 0.05）; compared with T1, the BRS of two groups in T2 were lower （P 〈 0.05）. Conclusion The BRS could be inhibit- ed by propofol and the inhibition was heavier because of increasing concentration of propofol. The inhibition was heav- ier in diabetes mellitus.
