老年2型糖尿病糖化血红蛋白、血脂水平与颈动脉粥样硬化关系

摘    要:目的:探讨老年2型糖尿病(T2DM)糖化血红蛋白(HbA1C),血脂水平与颈动脉粥样硬化(AS)的关系。方法:彩色多普勒超声测定颈总动脉中层厚度(IMT),66例老年T2DM分为T2DM-AS组和单纯T2DM组,测定HbA1C、甘油三脂(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)及脂蛋白aLp(a)]。同时与40例老年健康对照组比较。结果:T2DM-AS组和单纯T2DM组的HbA1C均高于对照组(P<0.05),T2DM-AS组HbA1C高于单纯T2DM组(P<0.05)。T2DM-AS组TC,TG,LDL-C,Lp(a)较单纯T2DM组和对照组升高(P<0.01)。但单纯T2DM组与对照组间上述指标无明显统计学差异(P>0.05)。T2DM-AS组IMT与HbA1C,TC,TG,LDL-C,Lp(a)水平正相关,与HDL-C水平负相关。结论:HbA1C可能促进了老年T2DM患者AS的发生,控制HbA1C,血脂水平有助于减少老年T2DM患者AS形成。

Abstract:Objective:To explore the relationship of glycated hemoglobin A1C(HbA1C) and blood lipids level with carotid atherosclerosis(AS) in elderly patients with tpye 2 diabetes mellitus(T2DM).Methods: Carotid ultrasound determined intima-media thickness(IMT),sixty-six elderly patients with T2DM were divided into group T2DM-AS and group T2DM.The levels of HbA1C、triglyceride(TG)、total cholesterol(TC)、highdensity lipoprotein-cholesterol(HDL-C)、lowdensity lipoprotein-cholesterol(LDL-C)and lipoprotein(a)were measured.Meanwhile,the results were compared with fourty elderly healthy persons.Results: HbA1C of both group T2DM-AS and T2DM were higher than that of control group.There were significant difference between the two group(P0.05).TC,TG,LDL-C,Lp(a)of group T2DM-AS were higher than those of group T2DM and control group.But there was no significant difference of above mentioned indexes between the group T2DM and control group(P0.05).In group T2DM-AS,IMT was positively correlated with HbA1C,TC,TG,LDL-C,Lp(a),IMT was negative correlated with HDL-C.Conclusions: HbA1C can aggregate the development of AS in elderly patients with T2DM.It could be beneficial for the precaution of AS in elderly patients with T2DM to control the levels of HbA1C and blood lipids.
