影响缩窄性心包炎近期手术疗效的因素分析

我院1974年4月至1995年8月共手术治疗慢性缩窄性心包炎74例。手术死亡率4.05%。术后并发症发生率32．43%。存活的71例中，出院时改善不明显有11例，占15．49%。17岁以下青少年、病程少于2年、术前心功能改善一个级别以上者，近期手术疗效好（P＜0．05）。术中应以中心静脉压和脉压差监测切除范围，术后防治低心排等并发症可保证手术疗效。

Analysis of relative facters affecting early operative results of constrictive pericarditis

74 patients with CP had received operations in our hospital from Apr. l974 to Aug.l995. The mortality was 4. O5%. The complication occurence rate was 32. 43%. In 7l survival patients,11 achived no significant progress wheri they discharged. 4t was about l5' 49%. The early operative result would be good if the patient,s age was under l7 years o1d, the duration of illness was less than 2years and the heart function was improyed more than one grade (NYHC) be fore the operation. In theoperation, the ablating range of pericardium shouldbe determined by measured CVP (central venouspressure ) and pulse pressure. Also a good result can be got by preventing and treating the postoperativecomplications such as low output syndrom.
