基质金属蛋白酶9与白细胞介素6和白细胞介素8水平与胎膜早破患者绒毛膜羊膜炎的关系研究

目的 探讨基质金属蛋白酶（MMP-9）、白细胞介素-5（IL-5）和IL-8对早产胎膜早破孕妇患绒毛膜羊膜炎的关系.方法 84例早产胎膜早破孕妇作为研究组,根据有无绒毛膜羊膜炎将其分为羊膜炎亚组（52例）与无羊膜炎亚组（32例）,以48例正常孕妇作为对照组.采用酶联免疫吸附法（ELISA）检查各组孕妇的血清MMP-9、IL-5和IL-8,同步取胎盘胎膜组织进行病理组织学检查.结果 研究组孕妇MMP-9、IL-6和IL-8均高于对照组,羊膜炎亚组血清MMP-9、IL-6和IL-8 [（248.36±121.34）μg/L、（436.37 ±167.78）μg/L、（612.54±216.3）μg/L]均明显高于无羊膜炎亚组[（116.56 ±67.87）μg/L、（269.39±169.67）μg/L、（349.35 ±168.34）μg/L],差异有统计学意义（P＜0.05）;绒毛膜羊膜炎患者血MMP-9与IL-5和IL-8含量呈正相关（P＜0.05）.结论 孕妇血中MMP-9、IL-5和IL-8的联合测定,可作为监测绒毛膜羊膜炎的重要指标.

Relationship between matrix metalloproteinase-9, nterleukin-6, interhukin-8 levels and premature rupture of membrane with chorioamnionitis

Objective To investigate the relationship between matrix metalloproteinase-9 (MMP-9), interleukin-6 (IL-6), interleukin-8 (IL-8) and premature rupture of membrane in pregnant women with chorioamnionitis.Methods The levels of MMP-9, IL-6 and IL-8 in maternal blood in 84 eases of premature rupture of fetal membranes (52 cases of chorioamnionitis) were detected by enzyme-linked immunosorbent assay(ELISA).Chorioamnionitis was diagnosed histopathologically after labor by detected placentas; 48 normal pregnant women were taken as controls.Results In patients with chorioamnionitis the levels of MMP-9, IL-6 and IL-8 in maternal blood were significantly higher than those of normal pregnant women and patients with non-chorioanmionitis(P ＜ 0.01) In chorio amnionitis the relationship between the levels of MMP-9, IL-6 and IL-8 is positively correlated.Conclusions The determination of level of MMP-9, IL-6 and IL-8 in maternal blood is a significant marker for monitoring ehorioamnionitis.
