胆囊结石高危患者的围手术期处理

目的:探讨高危胆囊结石的临床特点,提高围手术期治疗水平。方法:回顾分析我院自2002年1月~2007年5月收治的230例高危胆囊结石患者的临床资料。结果:胆囊结石并存心血管疾病187例,慢支气管炎肺气肿37例,肺部感染28例,糖尿病77例,两种及以上内科疾病70例。全组3例患者死亡,其余均痊愈出院。结论:对于高危胆囊结石的患者,通过积极治疗并存病,严格术前的准备以及加强围手术期的监护,能够降低术后并发症发生率,改善预后,减少住院时间和降低治疗费用。

Perioperative Treatment in Patients with High Risk Cholecystolithiasis

Objective:To study the clinical characteristics of the patients with high risk cholecystolithiasis and elevate the level of perioperative treatment.Methods:The clinical characteristics of 230 patients with high risk cholecystolithiasis from January 2002 to May 2007 were reviewed retrospectively.Results:One-hnudred and eighty-seven cases complicated cardiovascular disease,37 cases complicated chronic bronchitic emphysema,28 cases complicated pulmonary infection,77 cases complicated diabetes and 70 cases complicated two kinds of or more medical diseases.This series,three patients died,remaining patients recovered and were discharged.Conclusion:For the patient with high risk cholecystolithiasis,in order to decrease the incidence of postoperative complication and improve the prognosis,decrease the length of stay in hospital and cut down the cost for treatment,we should treat concurrent disease actively,strict preparation preoperatively and enhance perioperative monitoring.
