门诊药房用药差错分析

目的 汇总、分析门诊药房用药差错类型及原因,提出相应预防措施以杜绝药物差错的发生. 方法 系统回顾近几年门诊药房发生的用药差错,并归纳分析差错类型及原因,提出相应的预防措施; 结论 引起用药差错的原因很多,大致可以分为:处方差错、调配差错、给药差错、患者差错及其他差错.通过建立用药监测系统,规范开方、调配、给药操作流程,才能最大限度地杜绝差错的发生.

ANALYSIS OF MEDICATION ERRORS IN OUTPATIENT PHARMACY

Objection: To collect and analyze medication errors in outpatient pharmacy, to identify causes from results obtained, and to suggest feasible solutions to prevent them. Methods: Systematic review and analyze the types and causes of medication errors , suggest preventive solutions. Conclusion: Many factors can lead to medication errors, including prescribing errors、dispensing errors、administration errors 、patients errors and others. By establishing medication monitoring system, standardizing the procedure of prescribing、dispensing and administrating, medication errors can be avoided and prevented furthestly.
