皮下注射硼替佐米治疗多发性骨髓瘤的疗效及安全性的Meta分析

摘    要:
目的 评价皮下注射硼替佐米治疗多发性骨髓瘤的疗效及安全性。方法 计算机检索皮下注射硼替佐米治疗多发性骨髓瘤的随机对照试验,根据改良版Jadad量表评价纳入研究的质量,提取数据后,使用StataSE12.0进行Meta分析,依据系统综述和Meta分析优先报告条目(PRISMA声明)进行结果报告。结果 共纳入4篇文献,包括1 088例患者,皮下注射组与静脉注射组总反应率比较无统计学差异,但周围神经症[RR=0.70,95%CI(0.58,0.85)]和腹泻[RR=0.68,95%CI(0.52,0.89)]的总发生率差异有统计学意义(P<0.05)。结论 与静脉注射用药方式相比,皮下注射用药方式不影响硼替佐米治疗多发性骨髓瘤的疗效,但可以降低周围神经症与腹泻的发生率,具有更好的安全性。

Abstract:
Objective To assess the efficacy and safety of subcutaneous administration of bortezomib on treating multiple myeloma. Methods Randomized controlled trials of bortezomib on treating multiple myeloma were collected by the computer. After evaluating the quality of literature and extracting useful data,StataSE12.0 software was used to Meta-analysis. Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines were adapted for the report. Results A total of 4 RCTs were collected,including 1088 cases. There were no significant differences in overall response rate (ORR) between subcutaneous and intravenous bortezomib. However,peripheral neuropathy [RR=0.70,95%CI (0.58,0.85)] and diarrhea [RR=0.68,95%CI (0.52,0.89)] were reported in patients had a significant difference (P<0.05). Conclusion Compared with the intravenous group,subcutaneous bortezomib showed non-inferiority in treating multiple myeloma. In the meantime,the incidence of peripheral neuropathy and diarrhea could be reduced by subcutaneous bortezomib.
