胰岛素抵抗与糖尿病性腔隙梗塞的关系

目的：研究胰岛素抵抗（ＩｎｓｕｌｉｎＲｅｓｉｓｔａｎｃｅ，ＩＲ）与糖尿病性腔隙梗塞的关系。方法：对３４例非胰岛素依赖型糖尿病合并腔隙梗塞患者及３０例对照组患者进行了研究。分别测定了两组血胰岛素（Ｉｎｓ）、Ｃ肽（ＣＰ）、葡萄糖（Ｂｓ）、胆固醇（ＣＨＯＬ）和甘油三酯（ＴＧ）水平以及体重指数（ＢＭＩ）等指标。结果：糖尿病性腔隙梗塞组与对照组比较，其血Ｉｎｓ、ＣＰ水平明显升高，胰岛素敏感指数明显降低（Ｐ＜０．００１），其血ＣＨＯＬ、ＴＧ水平及ＢＭＩ亦升高（Ｐ＜０．０５）。结论：ＩＲ与糖尿病性腔隙梗塞有密切关系，ＩＲ是糖尿病性腔隙梗塞的独立危险因素之一，改善和治疗ＩＲ在预防糖尿病患者发生腔隙梗塞方面有重要作用

The Relationship Between Insulin Resistance and Diabetic Lacunar Infarction

Objective:To investgate the relationship between insulin resistance and diabetic lacunar infarction.Method:The blood glucose,insulin,C-peptide,trogluceride,total cholesterol levels and B M I were measured in 34 patients with lacunar infarction and N I D D M patients and 30 controls.Results: The results showed that the insulin and C-peptide levels in the diabetic lacunar infarction patients were significantly higher than those in the controls,and insulin sensible indexs decreased obviously(P<0.001).In addtion,the trogluceride,total cholesterol levels and BMI in the diabetic launar infarction patients were higher than those in the controls(P<0.05).Conclusion:The study suggests that insulin resistance play an important role in diabetic lacunar infarction.
