口腔黏膜癌浸润前沿细胞增殖的研究

目的研究口腔黏膜癌浸润前沿分级,检测增殖细胞核抗原(PCNA)在浸润前沿和非前沿中的表达。探讨口腔黏膜癌浸润前沿的结构和功能及其中细胞增殖对预后的意义。方法对30例原发口腔黏膜癌进行浸润前沿分级,采用免疫组化SP法分别检测浸润前沿和中心PCNA的表达。结果口腔黏膜癌浸润前沿(ITF)和非前沿WHO(1997)分级差异有显著性(P<0.05);浸润前沿PCNA的表达高于非前沿部分,差异有显著性(P<0.01);浸润前沿PCNA标记指数(LI)(P<0.05)与浸润前沿分级(IFG)总分呈正相关关系。结论口腔黏膜癌浸润前沿是整个肿瘤中增殖最活跃的部分。在浸润前沿中高PCNA表达可以反映肿瘤的高恶性度及高复发转移倾向。

The Study of Proliferation of Cells at the Invasive Tumor Front of Oral Mucosa Carcinoma

Objective To evaluate the value of the invasive front grading(IFG),the proliferative activity of cells at the invasive tumor front(ITF) and the center of oral mucosa carcinoma.To investigate the significance of they in the biological features of tumors and in the evaluation of prognosis.Methods The thirty samples of oral mucosa carcinoma were graded using invasive front grading.The expression of PCNA were evaluated at the ITF and the other parts of the same tumor by the immunohistochemical technique(SPmethod).Results There are significantly difference in the invasive tumor front(ITF) and the center of oral mucosa carcinoma about the WHO(1997) grading(P<0.05).The expression of PCNA(P<0.01),at the ITF were higher than those of other parts.The IFG score was significantly correlated with the expression of PCNA(P<0.05).Conclusions The ITF of oral mucosa carcinoma is the best active in proliferation of the whole tumor.The high expression of PCNA could be reflect the high malignant and the trend of recurrence and metastasis.
