DR与多层螺旋CT观测骶骨倾斜角的比较

摘    要:目的:探讨在多层螺旋CT矢状位重建图上观测骶骨倾斜角和腰椎曲度的可行性,以及两者间的关系。方法:选择106例拍摄有腰骶椎侧位DR片和CT扫描的患者,对DR片及重建图上的骶骨倾斜角和L3重力轴偏移距分别进行测量和比较 根据腰椎间盘有无突出(或膨出),将患者分成正常和病变两组,比较骶骨倾斜角的差异。结果:DR片和重建图骶骨倾斜角分别为34.18°±8.16°和31.50°±8.43°(P〈0.01) L3重力轴均位于骶骨岬前点后,偏移距分别为13.04mm和5.57mm,且偏移距均与骶骨倾斜角无明显相关。椎间盘正常组和异常组分别为32例和74例,DR片上骶骨倾斜角分别为30.94°±6.67°和31.69°±8.96°(P〉0.05)。结论:重建图上观测骶骨倾斜角与偏移距是可行的,但均比DR片测量值小,L3重力轴偏移距和椎间盘改变与骶骨倾斜角无明显相关性。

Value of MSCT and X-ray Film in Measurement of Sacrum Slant Angle

Abstract:Objective To investigate the value of MSCT and X - ray film in the measurement of the sacrum slant angle(SSA) and lumbar curvature. Methods Lateral views on standing position and MSCT scannings of the lumbosacral vertebrae were analyzed in 106 patients. (1)The SSA and the L3 gravity axis excursion distance were respectively mea- sured and compared between X - ray film and MSCT reconstruction imaging. (2) Two groups were acquired in the aspects of having intervertebral disc hermiation or not. The comparison of the SSA between the two groups was taken, Results (1) the SSA in the X - ray film and MSCT reconstnction imaging was (34.18± 8.16)o, 31.50 ± 8.43)0 respectively, with statistical difference( P 〈 0.01). There was positive correlation. The average excursion distance between L3 gravity axis and sacropromontory anterior point in the X- ray film and MSCT reconstruction imaging was 13.04 mm and 5.57 mm respectively. There was no correlation between the SSA and the excursion distance in this two kind of imaging (r= 0.062, 0.228). (2) There were 74 cases and 32 cases in the with and without intervertebral disc hermiation group, respectively, the SSA was (31.69 ± 8.96)°, (30.94 ± 6.67)°. There was no statistical difference between the two (P 〉 0.05). Conclusion (1) Compared with X - ray film, it was reliability to investigate SSA and excursion distance in MSCT reconstruction imaging, but with little statistic difference. (2) there was no necessary correlation between SSA and the intervertebral disc hermiation or the excursion distance.
