37例卵泡膜细胞瘤的临床分析

目的：探讨卵巢泡膜细胞瘤的临床特征、治疗和预后。方法：回顾性分析了新疆肿瘤医院1989年-2005年收治的37例卵泡膜细胞瘤的临床资料并查阅有关文献。结果：37例中，恶性泡膜细胞瘤1例；良性泡膜细胞瘤36例，其中4例伴有颗粒细胞瘤。患者年龄：50岁以上者56．7％，30岁以下21．6％，临床症状以盆腔包块为主诉就诊者28例，占75．7％，其次有腹痛者19例，占51．4％，阴道少量出血2例，25例（67．5％）合并雌激素刺激相关疾病，包括子宫内膜癌1例，内膜增生10例，子宫内膜息肉3例，子宫肌瘤11例。5例子宫内膜萎缩，8例合并腹水，25例行CA125检查，9例增高；9例增高全部为良性肿瘤。14例行患侧附件切除术，18例行全子宫双附件切除术，4例行全子宫双附件大网膜切除术，行PEB方案化疗6程，1例减瘤术后失访，5年生存率100％。结论：卵泡膜细胞瘤绝大多数是良性肿瘤，预后较好，临床常伴有雌激素增高的还可有腹水及CA125增高。

Clinical analysis of 37 cases with ovarian thecoma

Objective：To study the clinical caracteristics , treatment and prognostic factors of ovarian thecoma. Methods： From 1989 -2005 , a total of 37 patients with thecoma were retrospectively analyzed, and related articles were reviewed. Results： Among the 37 cases , one patient was malignant thecoma while 36 patients were benign thecoma ,4 of whom with granulose cell tumor. 56.7% of the patients were older than 50 and 21.6% were younger than 30 . The most frequent symptoms were pelvic masses and abdominal pain. 28 patients complained of pelvic masses （75.7 % ） and 19 patients complained of abdominal pain （51.4%）. Only 2 patients represented vaginal bleeding while 25 patients （67.5%） with diseases which associate with excessive estrogenic stimulation , including 1 endometrial adenocarcinoma , 10 endometrial dysphasia ,3 endometrial polyps and 11 myomas. Five patients with endometrial atrophy while 8 patients with ascites. Twenty five patients were evaluated blood serum CA125 , the CA125 were elevated in 9 patients with benign thecoma. Forteen patients received affected accessory resection ,while 18 patients received TAH BSO and omentorectmy followed by 6 course BEP chemotherapy. 1 patient received reducing - tumor operation and went to other hospital without following -up. The overall five -year survival was 100% . Conclusion： Most of thecoma cell tumors of ovary are benign and have good prognosis. The patients are often with components caused by excessive estrogenic stimulation, some of them are with ascites and increaed blood serum CA125.
