后路椎弓根钉内固定术治疗胸腰段脊柱骨折

目的探讨后路经椎弓根内固定手术治疗胸腰段脊柱骨折的疗效。方法对95例胸腰段脊柱骨折患者,先后采用5种椎弓根内固钉系统,单纯行后路减压、脊柱固定融合手术,观察伤椎椎体的复位率及远期椎体高度的丢失率以及神经功能恢复情况。结果伤椎体复位率(84.20±13.13)%,66例获平均18个月的随访,椎体高度丢失(21.0±12.68)%,损伤神经功能恢复良好。结论经后路椎弓根内固定手术治疗胸腰段脊柱骨折,具有操作简单、创伤较小、复位佳、神经功能恢复满意等优点,应首选。

Treatment of thoracolumbar vertebral fracture with transvertebral pedicle internal fixation

Objective- To study the efficacy of internal posterior trans-pedicular instrumentation for the treatment of thoracolumbar spinal fractures. Methods- 95 cases of thoracolumbar spinal fractures were treated by posterior fixation with 5 kinds of trans-pedicular instrumentations successively. Clinical (Frankle grade) and radiologic (correction of anterior corpus vertebrae height and correction loss after long term) outcomes were analyzed. (Results- ) The correction of anterior corpus vertebrae height was (84.20±13.13)%.The correction loss was ((21.0±12.68)%) and the thoracolumbar spinal fractures function was recovered evidently in 66 patients who were followed up for an average of 18 months. Conclusion- Transvertebral pedicle internal fixation has advantages of simple manipulation, less injury, good reduction, and satisfied neuronal function recovery, and should be preferred to treat the thoracolumbar spinal fractures.
